Flight Safety Investigation Report for paratrooper accident

The Royal Canadian Air Force’s Directorate of Flight Safety, as the Airworthiness Investigative Authority for the Canadian Armed Forces, has issued the final report in relation to the fatal parachute accident on June 17, 2019 during Exercise SWIFT RESPONSE in Bulgaria, which claimed the life of Bombardier Patrick Labrie, from the 2nd Regiment, Royal Canadian Horse Artillery based at Garrison Petawawa.

Flight Safety Investigation Report (FSIR) CC130J608 presents the findings and cause factors as it pertains to the airworthiness of the CC-130J Hercules aircraft systems, as well as recommendations and measures to prevent similar incidents from occurring in the future.
