The Transportation Safety Board of Canada (TSB) has released its investigation report (A18O0150) on the mid-air collision that occurred near the Ottawa/Carp Airport, Ont., in November 2018.
On Nov. 4, 2018, at about 10:10 a.m. EST, two aircraft collided near the Carp Airport located west of Ottawa, Ont. The first aircraft was a Cessna 150 with one person on board who was fatally injured. The second aircraft, a Piper PA-42 (Cheyenne III) with two occupants on board, was able to fly to the Ottawa Macdonald-Cartier International Airport, where it landed at about 10:30 a.m. EST.
The TSB conducted a limited-scope, fact-gathering investigation into this occurrence to advance transportation safety through greater awareness of potential safety issues.
